Yup. I did trim mine so I only needed to fold it once.

N. An applicant shall demonstrate competence with a firearm through any of the following:

1. Completion of any firearms training program that is approved by the department of public safety and that is conducted by instructors who are authorized by the department of public safety or who possess current national rifle association instructor certifications in pistol and personal protection and who submit to a background investigation, including a check for warrants and a criminal history records check.

2. Completion of any firearms safety or training course or class that is available to the general public, that is offered by a law enforcement agency, a junior college, a college or a private or public institution, academy, organization or firearms training school and that is approved by the department of public safety.

3. Completion of any hunter education or hunter safety course approved by the Arizona game and fish department or a similar agency of another state.

4. Completion of any national rifle association firearms safety or training course.

5. Completion of any law enforcement firearms safety or training course or class that is offered for security guards, investigators, special deputies or other divisions or subdivisions of law enforcement or security enforcement and that is approved by the department of public safety.

6. Evidence of current military service or proof of honorable discharge or general discharge under honorable conditions from the United States armed forces.

7. A valid current or expired concealed weapon, firearm or handgun permit or license that is issued by another state or a political subdivision of another state and that has a training or testing requirement for initial issuance.

So a copy front/back CCW should suffice.

I just called the UPS Store on Fair Oaks Blvd and he said $130 per card for the prints. I told him the other stores in town were around $25 for both cards. He said to call the other stores and they will all be the same. I won't repeat what I told him to do...Sheriff's Office here I come.

Anyone know how much postage goes on that envelope? I don't want to disarm to go into the post office if I don't have to.

Here's my update.

5/15: Email sent.
5/16: Received email notification.
5/19: Received packet.
5/20: Mailed packet.
5/24: Permit issued.

5/26: Received permit!!!!

This was the fastest turn around I have ever experienced!! Thank you Arizona for my early 45th BDay present!! You guys rocks!!!

I put 3 stamps on mine to be sure. It doesn't weigh any more than many other things I've mailed with one stamp, but the size may matter.

When I dropped mine off at the Post Office, the cleck weighed the envelope and asked me for $1.08.

That's because they leave a notice in the po box saying there's a package to be picked up at post office, I think thats what happend to me too, but luckly it was delivered a day after it was suppose too!

UPS here in mountain view want 19 for the first card and 18 for the second. Any info on Santa Clara Sheriffs Office? I wish they had a military discount for Memorial weekend!!!

Mailed application on 5/21. Permit arrived yesterday.

Allow me to add my experience. After I first saw this thread on 5/17, I posted to the thread asking for the e-mail address to request the application package.

I e-mailed my request same day: 5/17, Tuesday mid-afternoon
I received a confirming e-mail two hours later: 5/17, Tuesday late afternoon
I received the application with fingerprint cards : 5/20, Friday
I sent my application with fingerprint cards: 5/23, Monday
I just received my CCW Permit in the mail today: 5/31, Tuesday.

If not for the mail holiday yesterday, I would have received the permit in one week from the date I sent the application. As it is, they turned it in 5 working days from the day I sent my package, which allowing for transit time both ways by snail mail, I can only presume that they processed and sent the card on the day my application arrived. Certainly, no more than one day later! Man, that is really efficient.

By the way, in response to a number of questions:

1. I called and asked and they confirmed to fold the application and other documents to fit in the square envelope they provided, which was fingerprint card size. I just folded the paperwork twice into a square. I weighed my envelope and used three stamps, which was about $0.25 more than estimated.
2. I only sent a copy of my Utah CCW, which by the way, took almost 70 days to get!

Three cheers for a state that still recognizes our individual freedoms!

I have the application for almost 2 weeks now. It's still in my car trunk. lolz
I can't see the benefits of having the AZ ccw if my FL & UT already cover it (approve state).

Supposedly Nevada will recognize AZ CCW sometime July, if you dont already have a NV permit.

I have a NV permit, but thought that having the AZ permit will allow me to carry more than the two permitted handguns listed on my NV license.

The main benefit for me is Nevada, begining July 1st. It is cheaper than a NV permit, and you don't have to visit the state to obtain it. Another benefit for me is New Mexico.
These things may or may not apply to you and your circumstances.
Another benefit, for all of us, is supporting one of the most pro-gun states in the Union.
