We (the CERT teams in CA) do this every year. It's called Silver Dragon and it's mostly volunteers working with the local fire departments and other agencies like the Red Cross.
We distribute bags with disaster preparedness information to houses to simulate distributing Cipro for an anthrax attack. My HAM radio club always provides communications for the drill in my area. The only costs we incur are for the facilities (we typically setup at fire departments) and the cost of the bags/supplies. Plus of course the agencies that do participate do have paid employees, often like the Red Cross (also a lot of volunteers). Either way, it's not near what NYC spent.
