That modem you have might be only a few years old but it is only DOCSIS 2.0 capable. You need to sadly upgrade again to at least a DOCSIS 3.0 modem. COX should be using downstream and possibly upstream channel bonding which means that if you are in an area where there is an impairment on the 6 MHZ wide channel with channel bonding it will use either another 3 or 7 channels depending on how they have it configured.

Sure it could be the cable outside and it could be loose fittings or a grounding problem. Could be 1,000 different things that someone has overlooked but I would start here and see if it is more stable.
